The Dental Van has arrived at Tewantin SS and appointments are now available. This service is provided by the Sunshine Coast Hospital & Health Service. The Dental Van will be located in its new site near the School Hall off Goodwin St entrance, at the 'back' of the school.
Free general dental care is available to Queensland resident children who are:
- 4 years of age through to the end of Year 10.
- 2 -17 year olds who are eligible for the Child Dental Benefits Schedule (CDBS), where services are bulk-billed to Medicare (Please present your current Medicare card at the appointment)
- 0-3 year olds & students who have completed Year 10, who hold or are listed as a dependant on a valid Centrelink concession card (please present your card at the appt)
Appointments are available before & during school hours. Please call the staff on 0412 680 630.
With the school holidays 'just around the corner', the school has decided to email this information to parents directly.
To register your child/children for this service, please go to the link/attached PDF document, to download the new 'Child Oral Health Consent & Medical History' form. Please print out and complete one form for each child & return to the school Office as soon as possible. Consent forms are also available from the front office if you are unable to print at home. The Dental staff will then contact parents to make appointments for children to attend the Dental Van during school time.
If you would like a School Holiday appt – please call the Dental Van to make an appt & bring the completed form with you to the appt.
If parents wish to contact the Dental staff directly to organise an appt etc, please call 0412 680 630. NB – this is a DIFFERENT ph number to previous Dental van visits.
